Some of us have seen more of these fridges than others have.
I have noticed three versions of decal on the inside of the door. The first is black and white and is on an all steel cabinet circa 1929-31. I will post pictures when Ray reminds me to do so.
The second decal is the version that's described as the CK version. It's silver, has a blue GE logo on top. There's a text box below an orange ribbon. I will also post that here. There may be different versions.
The third decal I have seen is nearly the same as the above, but without the text box below. I have seen it on the ball top (CG).
What I am not sure of is if any of the CA's had decals. Would everyone please comment and look in their cabinets?
If you have a decal in your cabinet, post a picture of the decal and one of the cabinet. These fridges have gotten mixed and matched over the years. I am trying to satisfy my OCD.

I was pretty dang tired last night coming back from Chicago. Now I am reminding Travis to take a picture of the old DR decal. I want to see it! I have never seen it before. As far as the CG decal you have mentioned, I think that my 36 flat top also had that decal. At first I thought it had the whole decal but now that I look at it, the old outline is just for the top portion of the "whole" CK version that you have posted a picture of. I think that means that it too had this shortened decal. For me though, I am going to put the whole version on it just because I like the descriptive text.
Now, for the CA machines, I know we have some die hard CA fanatics here, heck I'll even sheepishly throw myself into that category. I also have no idea if they had a decal or not. My thought is that they MIGHT have had one since as you pointed out Travis, the last of the DR cabinets had one. So that would lead me to believe that the CA machines did have one. Until I see a CA cabinet with a decal on it, I won't know for sure. Now, I am going to put the full version "CK" style decal on my CA form A just because I think it will look neat and the text of the decal matches the function of the machine to a T.
As with anything this old, too much time has passed for these little details to be widely available. I'm just glad that we have nut jobs such as ourselves who actually care enough to help make reproductions and help out with the research involved with finding the differences between these glorious machines. Here is the decal on a two size painted cabinet that had a 40 fin DR2 with a short control with an open evaporator. I swear I have seen another version of a decal with a picture of a DR with an oven evap. Into my garage I go to hunt for that. Attachments:

I'll wait until Saturday so that Ray can supervise. There's a good chance that whatever I have seen it out there. It's not like I have inspected fridges and NOT brought them home.
I guess the super-freezer is in reference to the large capacity of the open porcelain steel evaporator. It's really makes the original oven types seem like crap.
I would think this decal would be appropriate for any DR after the oven evap models. I am still hunting for a CA decal.

Dang, that is a really nice decal. Or at least it was. I really like the "air current" arrows on it. The drawn picture of a DR just puts it over the top. Surely somebody has a better example that we can use to recreate this. Right?!

I was browsing through a totally different cities Craigslist ads (as I'm wont to do) and I just had to post this. First, take a look at the whole fridge and the cabinet badge. Looks like a garden variety CK form B from 1935 right. Well, the top is, that is for sure. Look at the interior shot of the door though. !!! I've never seen this inner door decal before! The picture is so grainy and blurry (of course it is). I am half tempted to have the seller take a closer picture of it. I seriously want to travel back in time and get spares of almost every pre war GE part... Maybe I need to contact the seller and see if we can get a better picture of this. I know of a few people that would want one!
